Established patients can use the Elation Passport link to message the team and schedule a follow-up or ongoing care appointment.

Our clinic is not contracted with any insurance plans, this includes Medicare, Medicaid, Medicare replacement/supplement plans and all commercial plans. This means that payment is due at the time of the services. Superbills can be generated upon request for self submission to your insurance along with chart notes and codes.
