Dry eye and ocular surface disease is multi-factorial. We start with a comprehensive appointment to determine the contributing factors of your specific case and then create a comprehensive and customized treatment plan. See above for all the offered treatments.

Most specialized dry eye treatments are not covered by insurances, therefore our clinic is not contracted with any insurance plans, this includes Medicare, Medicaid, Medicare replacement/supplement plans and all commercial plans. We do accept HSA and FSA plans. We do not file any insurance claims or accept payments from insurance. This means that payment is due at the time of the services. Superbills can be generated upon request for self submission to your insurance along with chart notes and codes. Check your policy to see if self submission of claims is allowed. Plan reimbursement is variable and typically less than the fair market value.

Dry eye disease is a chronic condition. Our goal is to improve your quality of life, your ocular comfort and reduce the burden of dry eye disease. Typically, several treatments are required early on to achieve meaningful improvements. Maintenance is typically recommended 1-2 visits/year to preserve your gains. For more complex cases, maintenance treatments may be recommended every few months.

Our office is small and does not have a receptionist. If you are looking for an appointment fill out our new patient forms and someone from our office will call you to schedule an appointment and answer any questions. If you are an established patient, please use Elation Passport Portal to contact our team.

Please message our team through your Elation passport at least 24 hours before your appointment if you need to cancel or reschedule. Cancellations or rescheduling requests made less than 24 hours in advance will be subject to a $100 cancellation fee.

Our office is dedicated to dry eye and ocular surface. We are constantly learning more about treatments and the factors that contribute to this disease state. Many patients who have “tried everything” come to our office and are pleasantly surprised to find multiple, effective treatment modalities remain. Patients often discover new and important components to their complex dry eye disease and with this comprehensive, customized approach, patients find an improved quality of life.

There are two identical towers. We are in the North tower on the corner of W Republican St. and 1st Ave W. closer to the Safeway.

There is affordable street parking surrounding the building. The parking lot is not available to patients. To make things easier please download the paybyphone app or use m.paybyphone.com to pay for parking.
